Smog Test Forms
The 3-part and 4-part Smog Test Only Invoices have really taken the TEST ONLY business segment by storm and are used throughout California. Forms measure 8 1/2″ x 7 5/8″ and are available in carbonless NCR paper. Each style features prompts for BAR-required information.

S3 Smog form
Form S-3 is a 3 part form printed on regular paper (white, yellow, pink). The form is printed in black with red accents and imprints with your company information in blue.
S7 Smog form
Form S-7 is a 4 part form in NCR paper with the last paper cardstock (white, yellow, pink, manila cardstock). The distinctive smog check logo is intermarked onto the form. The form is printed in black and red, and imprinted with your company information in blue.
Econo Smog form
The 3 or 4 part NCR paper Econo Form is simply amazing. This is a custom form printed in black ink to your specifications and still costs less than the S-3 or S-7 forms!! No extra charges
